The Rise Of AI Agents Is Breaking Access Governance

Traditional IAM systems can't track the non-deterministic behavior of autonomous agents, making intent-based oversight a security necessity.

Access governance was built for humans. Over the past decade, it has been stretched, awkwardly, to cover machines. Now, AI agents are arriving at scale, and will soon surpass both human and machine identities in number, speed and potential impact.

According to Deloitte’s 2026 State of AI report, 74% of companies plan to deploy agentic AI across multiple areas within two years. Most will do so without governance frameworks designed for this new class of identity. That lack of oversight is already producing incidents that traditional access reviews and monitoring cannot detect.

AI agents represent a new, riskier, identity category because they combine machine-level access with something machines have never had before: the ability to autonomously perform tasks, select tools and chain operations. Because their behavior is non-deterministic, an agent with identical permissions can act very differently depending on context.

This is not a prompt engineering or guardrails issue. Content filtering has its place, but it cannot answer fundamental questions:  Who is the agent? What is it authorized to do? How are deviations detected and stopped?

When Authorized Access Becomes a Liability

The emerging risk with agentic AI is not that agents will perform unauthorized actions. It’s that they will perform authorized actions, just not in the way or at the scale anyone anticipated. Governance doesn’t fail at the permission level, it fails at the intent level. Without the ability to evaluate intent, a governance model cannot distinguish an agent acting within its mission from one that has quietly drifted far outside it.

Consider a healthcare organization that deploys an AI agent to help clinicians retrieve and summarize patient information within an electronic health record (EHR) system. The agent is granted access to the EHR, permission to query laboratory results and the ability to generate summaries through a clinical portal. This is the kind of access a clinician might hold under role-based controls.

In practice, the agent retrieves patient histories, queries lab databases, pulls imaging metadata and accesses billing records to correlate treatment timelines. It synthesizes all of it into a single clinical summary.

From the IAM system’s perspective, every individual action is authorized. But when the security team reviews access logs, they find that the agent has been routinely pulling data across clinical, diagnostic and administrative systems that are normally governed under separate compliance boundaries. No individual action is flagged. The violation lies in the access pattern.

This is what makes agentic AI categorically different. The risk is not a single unauthorized action, but the machine-speed composition of legitimate permissions across systems that were never intended to be used together. The consequences range from regulatory exposure and breach liability to audit failure and loss of trust.

Governance for Autonomous Identities

As AI projects move into production, organizations need a purpose-built approach to securing and governing AI agents and their identities that spans three control elements:

From Inherited Permissions To Declared Purpose
When organizations deploy AI agents, they typically take the fastest path: either attaching the agent to an existing service account or granting it the permissions of whoever created it. This is implicit access, where the agent inherits privileges without any explicit definition of what it is supposed to do or actually needs. The result is predictable: over-privileged identities. An agent built to summarize clinical notes ends up with access to billing systems.

Explicit access means something different. Before deployment, governance frameworks should require a documented purpose statement that defines the agent’s intent, the systems it can interact with, and the operations it is authorized to perform. Security and identity teams can then map minimum privileges to the agent's intent, and nothing more. Access is scoped to operational purpose, not inherited from whatever account happened to be available.

Intent-Aware Policy Enforcement
Traditional IAM policies ask a simple question: Is this action allowed? For AI agents, that is insufficient. An agent may be authorized to read patient records, query lab systems and access billing data and still violate governance policy by doing all three in sequence as part of a single task.

Intent-aware policies evaluate how permissions are composed across systems, not just whether individual actions are permitted. They incorporate signals such as task context, system interaction patterns and behavioral sequences at runtime. When an agent’s behavior diverges from its declared intent, access can be restricted or stopped automatically before damage occurs.

Continuous Runtime Enforcement
In human identity governance, access reviews run quarterly or annually, which is a cadence suited to how human roles evolve. AI agents operate on a different timeline. Model updates, new tool integrations and changes to orchestration logic can silently expand an agent’s effective capabilities without any update to its access profile. Privilege drift accumulates between reviews.

Runtime visibility closes that gap by continuously tracking how agents use their privileges, detecting deviations from expected behavior and automating remediation when thresholds are exceeded. The goal is not to replace access reviews but to move beyond static permission snapshots to continuous verification of whether an agent is actually operating within its declared intent.

Governing the Third Pillar

Enterprise security and identity programs have spent decades refining access governance for human users and machine identities. AI agents are now a third pillar: autonomous, non-deterministic and capable of combining legitimate permissions into outcomes no one explicitly approved.

What’s missing isn’t another control, but rather visibility, understanding and enforcement of how permissions are actually used at runtime. AI agents don’t break access models by bypassing them; they expose where those models come up short. Until governance can evaluate AI agent intent and behavior, not just access entitlements, organizations won’t fully understand the agent attack surface or how to secure it.
